SacValley MedShare: Our Mission

To improve the quality and safety of healthcare across the North Central Valley, by sharing health information in a secure and efficient fashion between all providers, facilities and patients.

OUR VALUES

Communication plays a pivotal role in the efficient and effective management of patients, encompassing health and wellness maintenance as well as treatment. Healthcare quality often falters due to communication breakdowns between patients, providers, and other stakeholders, emphasizing the importance of shared information. Beyond benefiting patients, health information exchange (HIE) yields advantages for various participants. These benefits encompass enhancements in patient care efficiency, quality, and safety, cost containment in healthcare, improved overall healthcare delivery, compliance with standards at different levels, and the preservation of market share in an evolving healthcare landscape.

About SacValley MedShare

Underpinning our commitment to comprehensive healthcare coordination, the Development Committee, alongside its representative members from the categories mentioned above, also includes voices from County Public and Behavioral Health, Medical Societies, The Hospital Council of Northern and Central California, and an insurance payer.

As a valued member of the California Association of Health Information Exchanges (CAHIE), SacValley MedShare plays a pivotal role in a predominantly rural and mountainous region, where climate and seasonality often dictate travel possibilities. Effective communication is essential in our mission to seamlessly align primary care, specialists, hospital providers, and skilled nursing care facilities, especially for patients transitioning from acute care.

Our region encompasses the following counties: Butte, Colusa, Glenn, Lake, Lassen, Mendocino, Modoc, Napa, Nevada, Placer, Plumas, Shasta, Sierra, Siskiyou, Solano, Sonoma, Sutter, Tehama, Trinity, Yolo, and Yuba. Covering over 50,000 square miles, SacValley MedShare serves a population exceeding 2.6 million, making it the largest geographically contiguous region among all California Health Information Exchanges (HIEs), with one of the lowest population and provider densities.

Much of our population resides in medically underserved rural areas, while pockets of larger urban centers are scattered throughout the Sacramento Valley. These urban centers vary in size, with some exceeding 100,000 in population, yet most are smaller in scale. Our larger acute-care hospitals are either non-affiliated community not-for-profit entities or are part of prominent hospital networks, including Adventist, Dignity, Providence St. Joseph Health, and Prime. In smaller communities, acute care is provided by rural Critical Access Hospitals.

In 2021, SacValley MedShare embarked on an exciting partnership with KONZA, a national HIE platform known for its innovative technology and services. KONZA's track record of success is exemplified by its ability to facilitate the sharing of clinical information at the point of care while supporting transformative value-based payment models, care management, and data analytics. This partnership empowers patients, physicians, healthcare facilities, and providers across the country with unparalleled actionable intelligence. At SacValley MedShare, we are dedicated to fostering a healthier and more connected future for the diverse communities we serve, driven by our unwavering commitment to healthcare excellence and innovation.

At the end of the day, all we can do is hope that our decisions are coming from the right place; and with the right information, those decisions are easier and more accurate; and up-to-date health information can make that happen.
