SacValley MedShare (SVMS), a California Qualified Health Information Organization (QHIO), helps healthcare organizations improve care delivery by providing secure, reliable access to health information when and where it is needed.

Through California Care360™, SacValley MedShare’s Health Information Exchange (HIE) platform, participants have a modern, efficient, and secure way to access and exchange patient information. Around-the-clock availability helps authorized providers retrieve critical information when traditional healthcare facilities may be closed, including during urgent and emergency care outside a patient’s primary care setting.

By connecting primary care providers, specialists, hospitals, and other care partners, California Care360™ supports better care coordination through timely information sharing, access to results, coordinated treatment, and more informed clinical decision-making. These capabilities help reduce information gaps and give providers a more complete view of the patients they serve.

Direct Secure Messaging

SacValley MedShare offers Health Insurance Portability and Accountability Act (HIPAA) Compliant, web-based or Electronic Medical Records (EMR) integrated, secure communication between providers and facilities, eliminating the need for faxing and allowing for more efficient near real-time transfers of care.

Web-based portal

Access Health Insurance Portability and Accountability Act (HIPAA) compliant, web-based, real-time longitudinal patient medical record information from all participating members, including Medications, Allergies, Lab Results, Radiology Reports, Continuity of Care Documents (CCDs), and more.

EMR Integrated Exchange

Benefit from real-time, longitudinal, patient medical record information integrated within your electronic medical records system, which includes Direct Messaging and Web-Based Portal Access.

Collective Medical

Enhance care coordination and communication with other providers through our EDIE (Emergency Department Information Exchange) Product, resulting in reduced readmissions, unnecessary ED visits, cost savings, and improved patient outcomes. This product is offered at no charge to hospital participants.

care Alerts

Care Alerts provide real-time alerts or a daily digest file of your patient or member's admissions, discharges, and transfers for payers, hospitals, practices, and county agencies when these events happen at a participating member facility. Every notification is a moment of insight about a patient’s health, so each and every one is of critical importance.

Member File Return

Member file return allows the member to receive a longitudinal xml CCD of their patient's longitudinal record for consumption into their electronic medical records system or clinical data repository. This can also be delivered to the member via real-time JSON stream.

An electronic health record (EHR), or electronic medical record (EMR), is a digital version of a paper chart that contains all of a patient's medical history from one practice. They are mostly used by providers for diagnosis and treatment.

health information service provider (HISP)

A Health Information Services Provider (HISP) works a little bit similar to an email service company. They securely transport encrypted health information from one healthcare provider to another using Direct Secure Messaging.
