At SacValley MedShare, we believe in “transforming data from moveable to usable.” Through our partnership with NinePatch, we’re expanding our ability to support Whole Person Care by integrating Social Services, Behavioral Health, Public Health, and clinical data into a single, connected ecosystem. With advanced referral exchange, built-in consent tools, and real-time interoperability across platforms, we’re helping our members access meaningful information that’s not just shared—but truly usable, actionable, and impactful.
We’re launching a new Whole Person Care platform in Q4 2025 that will give members:
• Referral Information Exchange (RIE®) for seamless, closed-loop referrals
• SDOH screening tools and consent management
• Built-in analytics and outcome tracking
• Interoperability with EHRs, county platforms, and CBO systems
This launch strengthens SVMS's commitment to improving care coordination and equity by connecting the dots between clinical care and community-based support services. NinePatch helps our members close gaps in care and meet new data exchange requirements under California's Data Exchange Framework (DxF).
NinePatch’s Whole Person Care (WPC) Community Information Exchange (CIE) is a highly visual and intuitive second-generation care coordination and closed-loop referral platform that is leading the way in overcoming the challenges around creating an integrated Whole Person Care eco-system.
