Migration Process FAQ

Why is SacValley MedShare migrating to a new platform?

SacValley MedShare is transitioning to a new Health Information Exchange (HIE) platform called CaliforniaCare360™ to provide participants with a more modern, efficient, and secure experience when accessing patient information.

This migration ensures our network continues to support high-quality care coordination with improved technology and performance.

How does this migration benefit my organization and my patients?

The new CaliforniaCare360™ platform offers significant improvements that support faster workflows and better access to patient data. Key benefits include:

  • Simplified, user-friendly navigation
Easily find patient records, view clinical summaries, and access tools with fewer clicks.
  • Centralized access to patient information
A more complete view of the patient record is available in one place.
  • Improved patient identity matching
Powered by Rhapsody Identity EMPI, the system links patient IDs across organizations to reduce duplicate records and improve accuracy.
  • Streamlined login and single sign-on (SSO) options (where available)
Helps reduce password fatigue and provides quicker system access.
  • Enhanced reporting and analytics
Improved insights to support quality improvement, operational planning, and population health initiatives.
  • Advanced security features
Stronger protection of sensitive patient data and continued support for compliance requirements.

With CaliforniaCare360™, participants will have broader access to timely and accurate patient data, enabling better care coordination with greater efficiency, reliability, and safety.
